Currently leads app scheduling has questionaires in the lead app booking form , for patients to schedule. however the custom questions (screener questions) dont populate anywhere in the patient profile, however there is a button in the leads app profile to click that takes you to the patient profile chart where you can see med hx , demographic etc.

I want to request a button in the patient profile (prob next to notes area) where you can click and it takes you to the leads app intake form where you can see answers to the custom questions you set up in the intake forms for online scheduling. this creates a 2 way information retrieval that TC's and Docs can pull up on the fly without having to manually exit the profile, click marketing and analytics and then find the lead profile card.
